Also, will link chronically homeless with resources that will assist them with funding, medical care, Veterans benefits, SSI, mental health and other needs
How will I contribute
Conduct daily street outreach targeting the most vulnerable in the community and with a focus on helping people obtain a level of housing appropriate to their health, case management and psychosocial needs. Also, will link chronically homeless with resources that will assist them with funding, medical care, Veterans benefits, SSI, mental health and other needs.Conduct initial PATH mental health assessments to help determine urgent and long term mental health needsConduct a VI-SPADT (Vulnerability Index-Service Prioritization Decisions Assistance Tool) on all new clients to connect them to the proper intervention and services, record VI-SPADT results for internal trackingConduct and complete all necessary paperwork to track client progress (notes, services, goals, exits)Meet with established clients to review, evaluate and support completion of their goalsRespond to community requests for street outreach services and send follow up informationRespond to and join local law enforcement as needed regarding at risk or homeless individualsProvide transportation to community linkages and appointments as neededAssist the client with benefits establishment (GR, SSI, TANF, Veterans Pension, disability verifications) obtaining ID, birth certificate, social security cards, provide employment linkages and assistance, housing referrals.Assist with housing applications and advocate with prospective landlords.Build relationships and collaborate with local substance abuse treatment and mental health service providers.Provide on-going information, referrals, linkages and advocacy for other identified needs.Provide a high-quality customer service environment for all clients.Participate in staff meetings and assigned trainingOther duties as assigned.
What I bring
High School Diploma (or Equivalent) 1-year experience as a Peer Support and lived experience with homelessness and mental health issuesClass C Driving LicenseKnowledge of local organizations and resources. Mental Health principles, effective presentation methods, needs of the community and able to present to all age groups.Ability to prepare and conduct educational presentations, communicate effectively with groups and individuals both verbally and written, instruct and assist in obtaining mental health services and information. Develop program material and use effectively, establish and maintain working with a variety of agencies, organizations and individuals.Able to work occasional evenings and weekends and work in inclement weather and in unsanitary conditions as neededBilingual English/Spanish preferred.
