As an ACT Team Case Manager, youll work hands-on with individuals living with serious mental illness, helping them gain access to essential services like housing, healthcare, employment, and education all guided by person-centered planning.
What Youll Do:

Build individualized recovery plans that promote independence and community inclusion

Link clients to services like healthcare, housing, employment, and benefits

Monitor progress, provide crisis support, and coordinate care with providers

Support transitions from hospitalization to home and from dependent to independent living

Advocate, empower, and walk alongside people on their path to stability
