RN – Home Based Hospital Care (Finance)



Work Schedule :

60% FTE, day/evening, 8- or 10-hour shifts, every other weekend rotation plus required on-call responsibilities during the off-hours (evenings/nights) with a one-hour response time to travel to patient's home. Scheduled holiday rotation. This position is eligible for shift and weekend differentials and mileage reimbursement. Hours are based off the operational needs of the department. You will travel and work primarily in patient's homes located within a 30-mile radius of University Hospital; your assigned work location will be 301 S. Westfield Rd., Madison, WI.

Pay :

  • Additional components of compensation may include:
  • Relocation assistance may be available for qualified applicants.
  • Shift Differentials: $4/hour evening and $5/hour night as applicable for hours worked.
  • Weekend Differential: $4.25/hour as applicable for hours worked.
  • Overtime
  • On-call pay

Exciting opportunity to join our Home-Based Hospital Care (HBHC) team providing hospital level care in the comfort of the patient's home!

We are seeking a Registered Nurse (RN) to:

  • Provide hands-on acute care in the home setting for the Home-Based Hospital Care (HBHC) patient population requiring the use of advanced assessment, technical, teaching, and decision-making skills. Other skills utilized may include: IV administration, lab draws, medication administrations, wound care, utilization of remote patient monitoring and video visit communication with the patient/family.
  • Work directly with patients and families in the comfort of their home utilizing care plans, interventions and equipment that are modeled after an acute care inpatient unit setting.
  • Work in collaboration with the interdisciplinary team to ensure that patients receive exemplary care in their home.
  • Perform comprehensive assessments of physical, functional, psychosocial, and cognitive status of the HBHC patient utilizing interview observations and physical exam techniques.
  • Assess post-acute needs and ensure that appropriate referrals to other services are made. Interdisciplinary conferencing is a key component of this role's responsibility, and thorough and timely documentation in EPIC is required.
  • Preferred candidates will have inpatient nursing experience.

Qualifications
  • Bachelor of Science in Nursing (BSN) Preferred

Work Experience
  • 1 year relevant acute care or home health nursing experience Required
  • Experience in teaching self-care skills to patients and families Preferred
  • Recent, relevant home health experience Preferred
  • Experience using EPIC and virtual health platforms Preferred
  • Current IV therapy skills with knowledge of venous access devices and ambulatory pumps Preferred

Licenses & Certifications
  • Licensed as a Registered Nurse (RN) in the State of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Required
  • Current CPR/BLS Certification Required
  • Current motor vehicle liability insurance in accordance with the UW Health "Motor Vehicle Policy" (Policy 1.17) and (1) A valid Wisconsin driver's license and (2) Successfully passing a driver's background check and (3) Two years of driving experience Required
  • Relevant certification Preferred
