Your Responsibilities:

  • 🧠 Evaluate, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders in elderly residents
  • 📋 Develop and implement personalized treatment plans in coordination with care teams
  • 🖊️ Maintain thorough and compliant documentation of evaluations, plans, and progress notes
  • 🧑‍🏫 Educate staff and families on communication techniques and safe swallowing strategies
  • 🏥 Participate in interdisciplinary care planning meetings to support resident outcomes

What We're Looking For:

  • 📝 Active state licensure as an SLP in SC
  • 🏥 SNF experience is a plus, but not required
  • 🙌 Compassionate, adaptable, and committed to patient-centered care

The Speech Language Pathologist (SLP) evaluates and treats patients, communicates with families, physicians and other health team members and maintains documentation of services in the medical records. Conducts in-services and training for facility staff on an ongoing basis. Apply
