Job Title: Full-time Hybrid Outpatient Therapy Coordinator

Millbrook Healthcare - Lancaster, TX

Millbrook Healthcare is seeking a Full Time PT, OT, PTA or COTA to serve as our Outpatient Coordinator. We are seeking someone who is passionate about their profession and takes pride in the work they do. As Outpatient Coordinator you will be responsible for building and strengthening relationships with current and prospective outpatient referral sources by completing face-to-face meetings with physicians, office managers, referral coordinators, and individuals in the community. You will educate these individuals on the services our facility offers, discuss patient care, referrals, and express appreciation for existing referrals. As Therapist, you will be treating older adults in our skilled nursing facility as well as in the community.

Responsibilities:

  • Dual role as coordinator and treating therapist
  • Outpatient marketing to ALFs/ILFs in the community to generate OP referrals
  • Provide communication to families of rehab patients/residents for return to facility for OP services
  • Assist the Director of Rehab with coordinating community events, public relations campaigns, or specialty marketing campaigns
  • Manage outpatient therapy schedule to establish and maintain POC as well as coordinate with other therapists to reschedule any missed visits
  • Assistance with clerical work for orders, MD signatures and referral follow up for continuity of care
  • Provide therapy services to long-term care, skilled in-patient, and outpatients within the facility
  • Provide therapy services in the outpatients' homes, group home, ALF/ILF

Requirements:

  • Licensed Physical Therapist, Occupational Therapist, Physical Therapist Assistant or Occupational Therapy Assistant
  • Experience in outpatient marketing preferred
  • Exceptional interpersonal skills, including the ability to quickly build rapport
  • Willingness to become involved in local community events
  • Minimum of 1-year SNF experience

  • Exhibits a sincere desire to help patients and residents in their rehabilitation process
  • Superior interpersonal communication skills
  • Strong writing skills required for documentation of the care you provide
  • Exhibits knowledge and understanding of therapy practice
  • Possesses and conveys high degree of credibility and integrity
  • Enjoys team cohesion, is a team player, has a positive attitude and a strong desire to make a constructive impact on organizational excellence
  • Understands and accepts the unpredictable nature and needs of the therapy function in a busy rehab department
