Social Worker - Licensed Clinical Therapist (Palliative care) (Finance)



We are seeking a Social Worker (LCSW) to join the Palliative Care team. The role of the PACT Social Worker is to support PACT inpatients and outpatients in partnership with the interdisciplinary palliative care team. The PACT social worker partners with the primary disease / unit based social worker and psychosocial providers to facilitate family communication with the medical team.

The PACT social worker joins the Palliative Care IDT to engage in family decision-making and goals of care discussions, assist with developing an end of life care plan as appropriate, initiate / support hospice referrals and discharge coordination, and support transition to bereavement when indicated. The PACT social worker is responsible for maintaining EMR documentation, participates in division quality improvement activities, is able to participate in research activities if interested, and other duties as assigned. The Nemours Palliative Care Team (PACT) seeks a social worker to support Palliative Care inpatients and outpatients in partnership with the interdisciplinary palliative care team. The PACT social worker partners with the primary disease / unit based social worker and psychosocial providers to facilitate family communication with the medical team.

Responsibilities:

  • Partnership with PACT psychosocial partners: bereavement coordinator, chaplain, child life specialist
  • Partner with primary disease / unit based social worker and psychosocial providers
  • Facilitate family communication with the medical team
  • Bio-psychosocial-cultural-spiritual assessment for family
  • Engage in decision-making, goals of care discussions
  • Follow inpatient and in the outpatient settings
  • Documentation
  • Develop end of life care plan as appropriate
  • Initiate / support hospice referrals and discharge coordination
  • If hospice - consistent communication with the hospice social worker
  • Postmortem family conference liaison
  • Gift of Life Partner for PACT
  • Provide support to clinical staff in psychosocial aspects of care provision
  • Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee's supervisor.

  • Qualifications:
    • Master's degree required
    • LCSW with Delaware licensure required
    • APHSW-C (Advanced Palliative Hospice Social Worker - Certified) preferred
    • Competitive candidates will bring three or more years of experience working in multidisciplinary teams.
